LATEST TREATMENT METHODS OF ACNE SCARS IN COSMETOLOGY
Acne
affects 80% of people between the ages of 11 and 30.
Active breakouts are frustrating enough, but the scars acne
leave behind can cause significant psychological
distress and social impact.
The good news is that acne scars can be treated.
However, before treatment can start you first have to get rid of
any acne once and for all since new breakouts can lead to new acne
scars.
Depressed or atrophic scars fall into the following
categories:

Ice Pick Scars
These are deep, narrow (around 2mm wide) scars that look
similar to open pores. About 65–70% of atrophic scars are ice pick scars.

Rolling Scars
These are caused by damage under the skin. There are
usually wider than 4–5mm and lead to shadowing on the skin surface. About
15–25% of atrophic scars are rolling scars.

TCA CROSS
What
is TCA CROSS and how does it work?
TCA
CROSS is the chemical reconstruction of skin scars (CROSS) using
trichloroacetic acid (TCA). It is most often used as an outpatient clinic
procedure to treat atrophic acne scars. It's inexpensive, safe and efficient.
The
TCA CROSS procedure involves depositing small amounts of TCA at high
concentration (70–100%) onto the surface of the atrophic scar. This causes a local inflammatory reaction leading to
the formation of new collagen fibres.
The
aim is to improve the appearance of the scar by increasing collagen reflectance
and reducing shadows cast over scar depressions.
Complete resolution is unlikely to be achieved
with this treatment method alone.
DERMAPEN
TREATMENT

The Dermapen, Microneedling
treatment can provide drastic results improving the appearances of the scars.
The micro needles of the Dermapen create new collagen and elastin to be
generated and deposited called collagenesis, however the added benefit of new
capillary growth is also a result which can improve blood supply to the area
which helps create healthier existing skin as well as provide a healthy
foundation for the new skin. The physical nature of “skin-needling” breaks up
this fibrous and uneven scar tissue and stimulates the growth of new tissue.

Currently
there are ablative and non-ablative treatments offered which can damage the
epidermis and evaporate the skin leading to thinner epidermis problems. The
Dermapen keeps the epidermis integrity full intact while treating the area
which quickens healing time and causes less pain.
HOW MANY TREATMENTS?

While each patient is different and conditions will
vary, the typical treatment regimen will consist of 5-6 treatments.
THE FRAXEL LASER
The fractional CO2
(carbon dioxide) laser is an effective laser for the treatment of acne
scarring. The new “fractional” technology treats only a portion of the skin,
leaving healthy intact skin surrounding the treatment area. This allows for
fast healing and very safe treatments for all skin types.
How are acne scars treated by the
Fractional CO2 laser?

The fractional CO2 laser makes small
microscopic holes in the deeper layers of the skin. This process leads to the
generation of new, healthy collagen to smooth out the acne scars. Improvement
in acne scars can be seen in as early as one week following the Fractional CO2
laser treatment. Improvements in acne scarring will continue for up to 6 months
following the initial treatment. Approximately 30-70% improvement of acne
scarring is seen after one treatment, though results vary by the individual.
Most patients require only one treatment to reach the desired improvement.
What is the downtime for CO2 Fractional Laser Treatment for acne scars?

The fractional CO2 laser requires
approximately 4 days of downtime after the procedure. During this time, the
skin is healing from the wound that the laser causes to the skin in order to
stimulate the healing of acne scars. While healing in the 4 days following the
fractional CO2 laser, you will apply an ointment to the skin continuously,
which helps to stimulate faster healing.
Is the fractional CO2 laser painful?

The fractional CO2 laser procedure
for acne scarring is performing using only topical anesthesia. On the day of
your treatment, a topical numbing cream will be applied for one hour prior to
the procedure. During the treatment, you will be awake and experiencing only
very mild discomfort.
Is the fractional CO2 laser treatment for
acne scarring safe for darker skin types?

Fractional CO2 laser treatments are
safe for the treatment of acne scarring in darker skin types. All skin types,
including Asian, Indian and African-American skin, can be safely treated with
the fractional CO2 laser.
SUBCISION
·
Subcision is a minor surgical
procedure used for treating depressed cutaneous scars and
wrinkles. It is also called subcutaneous incisional
surgery.
·
Subcision is performed using a
special hypodermic needle inserted through a puncture in the skin surface. The
sharp edge of the needle is used to break fibrotic strands
that are tethering the scar to the
underlying tissue.
·
The release of the fibrotic strands
and new collagen deposition caused
by wound healing leads to cosmetic improvement of the scar. Subcision can be
safely performed in the outpatient setting and is usually well tolerated.
How is
subcision performed?
The procedure
of subcision is described below.
1.
The
area to be treated is cleansed to remove dirt and make-up

The
scar margins may be defined with a surgical marker, adjusting overhead lighting
to delineate depressions.
4.
A
tri-bevelled needle (number 18 or 20 gauge) or a Nokor needle is inserted at an acute angle adjacent to the scar with the bevel upwards and
parallel to the skin surface.
5.
The
needle is advanced through the dermis and
moved back and forth in a fan-like motion. A snapping sound is heard as fibrous
bands are transected in the deep dermis and deep dermal subcutaneous plane.
6.
The
needle is removed and squeezed circumferentially around the exit point to
prevent large haematoma formation
due to bleeding.
7.
Manual
pressure is applied to the wound for several minutes.
SOFT-TISSUE
FILLERS FOR ACNE SCARS
In
soft-tissue filler therapy, a filler such as Juvederm, Radiesse, Retylane
is injected directly into acne scars with a very small needle to
temporarily improve the skin's appearance.
Soft-tissue
fillers offer a temporary method of plumping and smoothing acne scars that is:

Minimally invasive
What to expect after the procedure
Patients may experience one or more of the following reactions
at the injection sites:
§
Bruising, which may last up to a week
§
Swelling
§
Redness
All soft-tissue fillers are temporary, lasting anywhere
from six months to two years, depending on the filler and type of procedure.
Some patients require maintenance treatments to achieve the best results.
